Not registered with a GP yet?

 

We welcome new patients who live within a 5 mile radius of the Practice. Registering is simple and easy. Give us a call to make your new patient check appointment.

During this new patient check we will take some basic measurements such as your height and weight and then take a brief medical history from you. It’s nothing to worry about, we simply want to get to know you and your health a little better.

The health check usually takes around 25 minutes or less to complete and once its done you will be registered with the surgery and be free to make appointments with any of our doctors and other clinical staff.

Please note if you are currently taking any medication on a repeat basis, you will need an appointment with the doctor to get these items set up on a repeat basis at our practice.

 

 

Documents to bring with you

 

Patients who wish to register with the practice need to provide us with their signed medical card which formally indicates that they wish to join this practice. Our GP's will also sign the card to acknowledge that they are happy to accept the patient onto the practice list. Any patient who cannot find their medical card will be issued with a HS200 form. Completion of this form will provide the information the Business Service Organisation need to accept the application and to trace previous medical records.

Non UK patients

Individuals from outside the UK who are now residents in Northern Ireland may register, however Health Board regulations dictate that a copy of your visa / passport is required to be submitted along with the completed HS22X.

Individuals holidaying or visiting relatives in the area may be seen by Doctors as a temporary resident. These services are available to individuals staying 3 months or less. Temporary residents cannot be referred to hospital for specialist treatment unless for an emergency. Temporary residents will not be given priority over registered patients for appointments, and will be offered the next available routine 48 hour appointment.
